Alzheimers Services of East Bay, Berkeley, CA

ASEB is growing and is looking for a Social Worker to join our team in Berkeley. ASEB is a non:profit community organization whose mission is to provide services to individuals with Alzheimers Disease and other related dementia; provide support for their caregivers; and educate and inform the general community about issues related to dementia care. ASEB provides day care, recreational activities, health care, and more.
JOB DESCRIPTION The Social Worker will be responsible for the pre:screening, assessment and admission of participants to the Adult Day Health Care and Alzheimers Day Care and Resource Center programs. The Social Worker will develop and maintain individual care plans for participants in the Adult Day Health Care program; provide supportive services to caregivers and community members; and will coordinate education, training and other projects. The Social Worker is a member of the multi:disciplinary team and reports to the Program Director. The Social Worker may maintain a working relationship with ASEBs consulting LCSW.The ideal candidate is reliable, dependable, works extremely well with other individuals, has excellent communication skills, pays attention to details, is organized, is patient, approaches life with positivity and enthusiasm, and has a genuine appreciation for elderly individuals, particularly those living with memory decline.

The Social Worker must be licensed as a clinical social worker by the California Board of Behavioral Science Examiner or a person who has a masters degree from an accredited school of social work
Be knowledgeable of the physical, physiological, emotional, and social needs of the elderly
Have a demonstrable aptitude for working with elderly
Experience in adult day support services and experience in the provision of counseling, support groups and assessments for individuals affected by dementia is preferred.
Duties of the Social Worker are:
1. Social Work
2. Interview and screen all referrals to determine the general appropriateness of the prospective participant.
3. Integrate ASEBs Family Support Program into the intake process as needed.
4. Provide referral for persons not appropriate for adult day program
5. Perform home visits as part of intake process and as indicated in care plan
6. Develop appropriate care plan for individuals admitted to program, monitor implementation, document progress of care plan and revise as necessary, on a quarterly basis at minimum.
7. Provide counseling, support and intervention to participants and caregivers in the areas of behavioral management, caregiver stress, coping strategies, and other issues as needed.
8. Educate families regarding symptoms of dementia, possible resistance to day care and safety issues related to cognitive impairment.
9. Discuss long:term care plan and schedule a discharge meeting with families and Program Director.
10. Serve as a liaison with the participants caregiver and other community services involved in care and with the multi:disciplinary team as needed.
11. Report suspected verbal, physical, fiduciary, and/or sexual abuse to appropriate monitoring agencies
12. Monitor participant attendance and case mix and center daily attendance
13. Facilitate monthly caregiver support group.

a. Obtain complete enrollment
information/paperwork on participants, including establishing daily rate, billing agreement, and subsidy application.
b. Produce accurate and timely end:of:month reports, including flow sheets.
c. Maintain and update medical emergency contact file
d. Attend meetings as necessary
e. Assist in the on:going development of simpler intake/admissions process.
